Prognosis Ikigai

Prognosis Ikigai was born from a defining moment in 2023, when I traveled to Japan to speak at the World Trauma Congress. Before the trip, I was given the book Ikigai: The Japanese Secret to a Long & Happy Life. Ikigai is a Japanese concept often translated as ‘life’s purpose’ or ‘the reason you get up in the morning.’ As I learned more, I discovered that ikigai resides at the intersection of what you love, what you’re good at, what the world needs, and what you can be paid for. Standing in the Arashiyama Bamboo Forest, looking up at the soaring stalks above me, I realized I had found my own professional ikigai—and was profoundly grateful. That moment inspired this work. Today, the healthcare profession is facing unprecedented challenges, including increasing levels of burnout and moral injury, making it harder than ever for clinicians to find meaning, connection, and sustainability in their work. Through coaching, consulting, public speaking, and my podcast, Prognosis Ikigai exists to help healthcare professionals reconnect with purpose and fulfillment, even within a strained and complex system. Loving your work—and living your purpose—is still possible; it simply takes intention, insight, effort, and a little help along the way.

Dr. Nicole Fox

MD, MPH, FACS, CPE

With more than 20 years of experience in healthcare, Dr. Fox is a trauma surgeon, surgical intensivist, and physician executive who operates at the intersection of clinical excellence, leadership, and innovation. Dr. Fox graduated from Franklin & Marshall College with a BA in political science. She received her medical degree from Rutgers New Jersey Medical School along with a master’s in public health/health administration from the Rutgers School of Public Health. She completed her surgical residency at Cristiana Care in Newark, DE and her surgical critical care fellowship at Cooper University Health Care in Camden, NJ.

Her career has been shaped by frontline experience in high-acuity environments and a deep commitment to designing systems that enable both exceptional patient care and workforce sustainability. Dr. Fox has built and led enterprise-level initiatives across academic healthcare organizations, with a focus on clinical operations, quality improvement, utilization and documentation strategy, patient experience, and physician development. She is particularly known for her ability to identify structural barriers and create scalable solutions—supporting physician performance, engagement, and retention while aligning clinical care with organizational strategy. Her work centers on creating cultures of trust, accountability, and professional fulfillment in environments where stakes are high and change is constant.
